Please fill out the registration form to confirm your student athlete is interested in being apart of the WASP. Membership designation will only apply when our facility is built. Choose the type of registration, and then pay... you can pay in full or select manual payment to work out a payment plan or find a sponsor. If you are choosing One-on-One, Clinics, or Camps, you are not required to complete uniform section.
I give permission for my child to participate in all activities of WASP. I acknowledge that the nature of the activities may expose my child to hazards or risks that may result in my child’s illness, personal injury, or death and I understand the nature of such hazards and risks. I accept these hazards and risks. I further agree to hold harmless WASP and its governing board, officers, employees, and representatives, in their individual and official capacities, from liability for the injury or death of any person(s) and damage to property that may result from Participant’s negligent or intentional act or omission while participating in tryouts, practices, or games. To the best of my knowledge, there are no physical or other conditions which will interfere with my child’s participation.
No Liability Insurance: I understand that WASP does not provide, nor is any member of my family covered by, any policy of liability insurance which would otherwise serve to compensate members of my family in the event of an injury, expense, loss of compensation, loss of service or other damage (general or special) which I or any member of my family may experience as a direct or indirect result of the use of services, facilities, instructions, or premises of WASP, its officers, agents, representatives, coaches, volunteers, or employees.
Photography and Video Release: I give WASP the authorization to photograph my child for any WASP related use, such as news media or promotion, website, and newsletters.
WASP Refund Policy: I acknowledge and consent that may withdraw their student athlete from any WASP activities with full refund within 24 hours of purchase. After the 24-hour window has passed, WASP can offer a credit to be utilized for another service that we offer. Credits must be utilized within 6 months of purchase or the start date of membership. Otherwise, the WASP will not provide a credit or refund (per WASP Bylaws ARTICLE V, section 3).
Agreement Consent: I consent that this document constitutes the entire agreement between WASP and the undersigned regarding the subjects covered hereby. All previous agreements, oral or written, are superseded and there exist no further oral or written representatives, promises, assurances, or statements of any kind affecting this agreement except those which are expressly set forth in this document. I acknowledge that the information I provided is true and accurate. I consent to reading WASP bylaws, policies, and handbooks once accepted into the program. I HAVE CAREFULLY READ AND UNDERSTAND THIS AGREEMENT, BY CLICKING SUBMIT I AM SIGNING THIS AGREEMENT.
NOTE: Tournaments are $50 plus and an extra $25 hotel fee for team rooms per tournament if needed, and/or for designated tournaments starting fall season. Tournament fees are due at the binging of each month.
